Carolina Youth Development Center has integrated the Transition Framework (TF) into all components of our system of care.
CYDC maintains an inter-dependent approach to the way we manage the agency, deliver services and collaborate with the community to deliver services to children and their families. The Transition Framework is the vehicle we have chosen to accomplish this goal.
As we go through life, we cannot move ahead without leaving something behind. To move ahead, our goal is to let go of the person we used to be and find the new person we will become in a new situation.
How did we get here?
- We have created and published a handbook, distributed to all employees, which provides all guidelines for the integration of the Transition Framework into CYDC’s operations.
- We provide Transition training to all CYDC staff and Board members.
- We incorporate the Transition Framework into all CYDC programs.
- We incorporate the Transition Framework into our plan of care for each child.
- We expanded our services to incorporate a “Mentor for Life” Program.
- We established an inter-agency council representing internal and external stakeholders.
- We conducted a comprehensive assessment of the board, staff, youth and partners to determine the impact of the TF.
We have achieved the goals outlined above, and we document the evidence of the impact of the TF periodically.
The Transition Framework is an initiative funded by the Andrus Family Fund.
